Flexible weekday PRN opportunities with Proactive MD

Are you a Nurse Practitioner looking for meaningful clinical work with the flexibility of a PRN schedule?

At Proactive MD, we believe healthcare works better when providers have the time and freedom to focus on their patients—not productivity metrics. Our workplace health centers provide direct primary care to employees and their families, creating an environment where providers can build relationships, practice broad-scope medicine, and make a meaningful impact on the health of the people they serve.

Why Proactive MD?

More time with your patients.
Our providers spend an average of 30 minutes per patient visit, giving you the opportunity to listen, understand the bigger picture, and provide thoughtful, comprehensive care.

Practice medicine—not the numbers.
There are no RVUs, productivity quotas, or volume-based measures. We aren't a traditional fee-for-service practice. Our focus is on improving patient health, satisfaction, and engagement—not seeing as many patients as possible in a day.

A true primary care experience.
You'll have the opportunity to practice broad-scope primary care, including preventive care, health education, chronic disease management, and acute/episodic care, while developing meaningful relationships with your patients.

A supportive team environment.
Our providers work collaboratively with Medical Assistants, Patient Advocates, and clinical leadership to provide a comprehensive approach to patient care.

Weekday flexibility.
This is a PRN weekday opportunity, making it a great option for an NP looking to supplement an existing schedule, maintain clinical flexibility, or simply practice in a different kind of healthcare environment.

Voluntary Self-Identification of Disability
Voluntary Self-Identification of Disability Form CC-305
OMB Control Number 1250-0005
Expires 07/31/2029
Why are you being asked to complete this form?

We are a federal contractor or subcontractor. The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years.

Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. If you want to learn more about the law or this form, visit the U.S. Department of Labor’s Office of Federal Contract Compliance Programs (OFCCP) website at www.dol.gov/ofccp.
