JOB SUMMARY
The mission of the Patient Engagement Specialist is to engage high risk patients with chronic conditions, patients with external and internal barriers to care, and the new hires of an employer population. The Patient Engagement Specialist will strategically engage employees and their dependents before, during, and after visits to the center in order to ensure we are providing personalized, informative, and compassionate care. The Patient Engagement Specialist role is supported by and reports to the Regional Director of Client Relations. The individual will work closely with enterprise Client Relations Managers, Marketing, Reporting and Client Representatives.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Patient Engagement
· Educate and promote health center services for the assigned population with the goal of scheduling new patient and follow up visits at the health center to increase overall utilization of services.
· Works in coordination with the Client Services team to analyze data and develop outreach campaigns. The Patient Engagement Specialist is responsible for the execution and success of these campaigns
· Represent the health and wellness center at internal and external meetings and events.
· Conducting events on-site as often as weekly to engage the employer population.
· Reaching out to patients via phone, email or mail educating patients on the services of the health and wellness center.
· The ability to creatively outreach and engage to employees in rural areas where there is little to no technology.
Patient Education
· Be knowledgeable about and provide information to patients regarding available community services that could enhance their care.
· Understanding the services and scope of practice delivered at each health and wellness center.
· Basic healthcare knowledge related to basic chronic conditions (ex. Diabetes, Hypertension, Hyperlipidemia)
Client Engagement & Support
· Provide support as assigned to small to medium employer clients through direct relationship management, report review, experience resolution and execution of engagement initiatives.
· Provide support to medium to large employer clients with direction from the assigned Client Relations Manager focusing on planning and execution of engagement initiatives to drive utilization of Proactive MD services and ensure attainment of Performance Guarantees and/or Client Strategy Goals.
REQUIRED KNOWLEDGE, SKILLS, & ABILITIES
Required:
· Bachelors degree in associated field--Health Promotion, MPH or CHES preferred but not required
· 2+ years of experience working in healthcare setting
· Ability to communicate effectively and maintain working relationships with people from diverse backgrounds
· Skilled in health-related programming and engagement strategies
· Knowledge of the healthcare industry
· Ability to prioritize needs and plan work accordingly
· Knowledge of HIPPA confidentiality requirements
· Knowledge of basic medical terminology
· Electronical Medical Record Experience (Elation, Epic, Cerner, McKesson)
Preferred:
· Master of Social Work
· Bilingual (Spanish)

WORK ENVIRONMENT & PHYSICAL REQUIREMENTS
This job primarily operates in a professional office environment. The physical demands to perform the essential functions of this job include:
· Effective communication, mobility, ability to operate office equipment and travel intermittently throughout the day.
· Must be able to remain in a stationary position at their work area for prolonged periods.
· Additionally, the employee will need to be able to lift/move/store individual 15-20 pound boxes and items such as office and medical supplies and equipment at events.
POSITION TYPE & EXPECTED HOURS OF WORK
This role will be expected to work a minimum of 40 hours/week as directed. Typical workdays are Monday through Friday, 8:00am to 5:00pm. This role is considered an exempt position. Evening and weekend work are infrequent but may occasionally be required as business needs dictate. This is a non-management role with professional development and growth opportunities within Proactive MD.
TRAVEL
Travel is primarily local during the business day. Travel required to assigned employer sites up to 15 days out of the month. Travel needs vary by client need but would primarily be within state of candidate residence, unless otherwise discussed and agreed upon during hiring process.
